$25 helps provide essential supplies such as exam gloves, bandages and other basic care materials for a patient visit.
$50 covers a patient visit to our walk-in acute care clinic.
$100 helps provide diagnostic testing and treatment for an acute illness or injury.
$250 covers a patient visit to the medical clinic OR helps provide physical rehabilitation services that restore mobility and independence.
$500 helps a patient manage a chronic condition such as diabetes or hypertension for several months.
$1,000 helps provide the lab tests and diagnostic services our patients need.
$2,500 helps provide a day of comprehensive care across Church Health’s clinics.
